| At many times in your graduate career, you will be asked to fill out a form. Janice Lunsford can help you decipher which forms you need when, and most forms (including those for M.S. students) can be found on the Graduate School web site. On this page, you can find some of the more commonly used form.

Annual Progress Report |
|
ANNUAL COMMITTEE MEETING 
As described in the Student Handbook, every student, regardless of source of financial support, must submit an annual committee report each year. You need to have a committee meeting by March 1 of each academic year, with the complete report to be turned in by March 15. First-year students are expected to solicit comments from their temporary advisor and their three rotation mentors for this report.
Faculty section: The Committee Report form is to be completed by your advisor, with feedback from each committee member for each section of the form. This should be a thorough and constructive summary of what was discussed at the annual meeting. The form must be approved by each committee member and given to you within two weeks of the annual meeting.
Student section: Read the faculty responses and sign the form as the last member of the team that is working together toward your training goals. Then, append a one-page response to these evaluations, including a summary of your progress and projected work for the coming year. Turn this in to Janice along with your current CV with information on presentations given, manuscripts submitted and/or published, proposals written and/or funded, courses taught, awards received, outreach, and mentoring. If you have scheduled a defense date in that spring semester and you are confident you will not need funding during the summer of that year, you are not required to submit a progress report.

PRELIMINARY DOCTORAL PROGRAM OF STUDY 
Take this form to your first committee meeting.

FINAL DOCTORAL PROGRAM OF STUDY 
This form confirms that you have completed all your course requirements. The Graduate School must be notified at least two weeks before your orals; you must e-mail the Graduate Coordinator or Janice Lunsford with the title, time, and place of your oral presentation. The Graduate School will send the final version of this form to your adviser and it will be signed by your committee at your orals.

APPLICATION FOR ADMISSION TO CANDIDACY DOCTORAL DEGREE 
This form is submitted after you pass both your written and oral qualifying exams.

APPROVAL FORM FOR DOCTORAL DISSERTATION AND FINAL ORAL EXAMINATION 
Take this form to your final defense. This PDF includes the electronic thesis and dissertation (ETD) submission approval form. The Graduate School must be notified at least two weeks before your orals; you must e-mail the Graduate Coordinator or Janice Lunsford with the title, time, and place of your final defense. |
